Diagnosis of CPP

It is very important for male patients to have a thorough diagnostic workup. This includes a careful history and a physical exam, including a digital rectal exam to check the prostate gland. Urine tests are ordered and include a urinalysis and a urine culture to be certain that there is no blood in the urine and no urinary tract infections. Another test which is commonly performed is a cystoscopy—a look into the bladder with a lighted tube. In approximately 5 percent of men with CPP, there are characteristic ulcers, referred to as Hunner’s ulcers, located in the inner lining of the bladder, and revealed through a cystoscopy.

Another diagnostic test is hydrodistention of the bladder, or filling the bladder with sterile water to determine the bladder capacity. Hydrodistention is performed under general or regional anesthesia. This workup will help rule out other medical conditions that cause pelvic pain and will help to confirm chronic pelvic pain as a diagnosis.
